RELEASE OF LIABILITY AND WAIVER

THIS RELEASE AND WAIVER is executed on [DATE] by [PARTICIPANT NAME] ("Participant") in favour of [ORGANIZATION NAME] ("Organization").

1. ACKNOWLEDGMENT OF RISK

I, the Participant, acknowledge and understand that:

1. The activity of [DESCRIBE ACTIVITY] involves inherent risks, including but not limited to:

2. These risks cannot be eliminated without jeopardizing the essential qualities of the activity.
3. The Organization's staff and instructors are qualified, but accidents can still occur.

2. ASSUMPTION OF RISK

I voluntarily and freely choose to participate in [ACTIVITY] and I ASSUME ALL RISKS, both known and unknown, whether caused by negligence of the Organization or otherwise, that may result in personal injury, property damage, or death.

3. RELEASE AND WAIVER

In consideration of being permitted to participate, I hereby:

1. RELEASE, WAIVE, AND DISCHARGE the Organization, its owners, officers, directors, employees, agents, volunteers, and contractors from any and all claims, demands, damages, or causes of action arising from my participation.
2. AGREE NOT TO SUE the Organization for any injury or damage I may suffer.
3. INDEMNIFY AND HOLD HARMLESS the Organization from any loss, damage, or expense (including legal fees) arising from my participation or my breach of this Agreement.

4. MEDICAL TREATMENT

• I consent to emergency medical treatment in the event of injury.
• I am responsible for any medical costs incurred.

5. INSURANCE

• I understand the Organization does not provide health, accident, or liability insurance for me.
